Continuing care homes are publicly funded facility-based accommodations that provide care, including a range of health and support services appropriate to meet residents’ assessed needs.
Staffing arrangements differ between Continuing Care Homes, but your healthcare team is accessible in all types of Continuing Care Homes, either onsite, on-call or virtually.
What is a Continuing Care Home Type A (formerly Long Term Care)?
This environment provides onsite registered nurse and/or registered psychiatric nurse care, assessment and/or treatment 24-hours a day. Licensed practical nurses may also be onsite in addition onsite personal care and support provided by health care aides. Continuing Care Home Type A may also have a secure space. Some sites may have specialized programs and services available for residents with complex clinical or complex functional care requirements (e.g., rehabilitation).

Scheduled & Unscheduled Professional & Personal Care with Registered Nurses & Health Care Aides
Is for individuals who may:
- have highly complex, unpredictable medical needs including chronic disease management
- experience serious changes in healthcare needs requiring unscheduled registered nurse assessment and intervention
- have a need for complex medication management and other treatments delivered by an registered nurse
- have health conditions requiring rehabilitation or dietary professional assessment and intervention
- have complex nutritional needs requiring diet or texture requirements
- have unpredictable behaviour, including increased safety risks to themselves or others
- require extensive assistance with all aspects of daily activities including grooming, hygiene, eating, transfers, mobility, etc.
- be unable to use a call system to get help
Who will provide your care?
- Health care aides are onsite 24 hours a day to provide support and personal care according to your care plan
- Registered nurses onsite to support care needs.
- Depending on the needs of the resident, other healthcare professionals such as occupational therapist, pharmacist, recreational therapist, etc.) are available
- Medical care is provided by either the resident’s physician/nurse practitioner or a site designated physician/nurse practitioner that visits the site regularly.
